aA logistic regression model was used to correct the crude OR for the maternal confounders ‘maternal age’, ‘parity’ and ‘ethnicity’.

cThese congenital malformations could not be divided into major or minor malformations because they were coded as general non-specific categories of malformations e.g. ‘other congenital malformations of central nervous system’.
